Urgent: Need to make a transaction form

Ok so i have this project in which i have to make a movie renting database
Here is what i have made so far.. Attachment 385 and i have tried 5 times to make a proper working transaction form but i have failed..



To give you guys an example of what im trying to make the transactions form like is here Attachment 386. like i have list of all the available movies and a list of all the renters in the database and then i have to choose them and then when i click issue movie then transaction is made where i specify dates and the price is calculated.

i know my project is kinda a mess but please help me with this .. my project is due on sunday

ok as i did not get any reply for that.. can anyone tell me how to calculate the price as it depends on the selection in the list box of the movie because the price per day field is in the selection and it needs to be multiplied with the duration which is on the transaction form itself.

any help?
