I have a layout of customer service Marking guide and scorecard.



I want to create a access database for them So many fields like CallID, agent names(drop down list) manager, signature etc. also a 10 questions to mark it how they performed on call marking like 0% , 5% based on the call conversation and YES or NO options to select.

I want to create a database and probably form and Subforms also, after filling the form I need a button when I click that button automatically send an email to the agent. In that email I need to send a feedback report whatever we fill in the report.

Can anyone please help me how to create a button to send report directly to the agent. Also which columns should I put forms and subforms. .

What is the best way to create a database based on attached screenshot...

Can I wrote any queries to create it in the database ?
