Same conditional formatting rule, different results

I have the same conditional formatting rule on two forms in two different databases, one form displays the formatting correctly, the other does not display the formatting at all. Why?

See before and after screenshots. Both forms' On Current events are empty. Yes, I hit apply before expecting anything.

Ok so I realize I didn't make a rule for dates 90 days past due in the screenshots above for my right hand form. I just added the exact same rule and still no formatting.

Not sure if this has anything to do with it, but also any time I try to add a custom button (not part of the button wizard's function list upon button creation), I get an ActiveX/OLE object communication error.

So I guess my messed up form was, well, messed up. Possibly corrupt. I just deleted it and recreated it and now all the formatting and code works fine. Merp.
