need to create sub-forms with multiple entries

Hi

So I need to build a database regarding a single hospital visit of a patient.


What I need is then a day by day breakdown of the admission.
For eg on the form if the patient is admitted from 3/12/2013 to 3/15/2013, the number of days of hospitalization would be 4.
Then I need a connected form which can enter same data for day 1, day 2, day 3 and day 4.
The number of days would differ based on the number of days hospitalized.

I would appreciate any advice on how to go about this!

thank you but I tried making a db with this but i am unable to do so...
i have a db from a friend, and i want to build a similar format , but the db is too complex for me to understand.
Is there any way i can send you the db so u can make sense of what i need to do!
