Hello All, I am new to MS ACCESS databases, my organization (not for profit) is providing home base care to people living with HIV, we are currently using paper based forms to collect this information and we would like to switch to a database. The information collected on the forms include: Provider name, Provider number, Supervisor name, Date etc. on the client side it's client number, age, sex and the services and supplies given. The services include. Family planning counseling, Nutrition counseling and others, as for supplies, they include, family planing pills, condoms, gloves, water purification reagents etc.. I am trying to make a database that will collect all this information per client for every visit they receive from the provider and what they were given in terms of services and supplies. Can any one help me on how to start this and finally finish it ??? am really running short of time... Thanks in Advance..
