    Access Form help

I have been working on this for two days...I need to build load sheets..I have one sheet showing all product recieved... I want to use a form where I can enter the trailer info and dock door info and then have a subform where I can choose Yes or No on the product recieved sheet I have tried different ways and queries and the closest I got was a list with everything on every load...I recieved a new laptop and lost all my tutorials and cheat sheets...can someone point me in right direction to create this?

Ihave attached the spreadsheet I uploaded into access for my table

    Thank you in advance

I have added a spreadsheet of what I want the final report to look like
