Forms and automatically creating a coressponding scorecard

Good Afternoon,



I'm constructing a proposal database that will store, obviously, proposal information for different programs and each with it's own scoring system.

I would like it to be so that a corresponding scorecard will be constructed for each proposal and include all of the scoring categories. Can someone help show me how that is done?
