Query - Hospital data problem - please help!

I am having a problem creating a query that will search for what I want.

What you have attached is a spreadsheet. It is difficult to read because you typed many words within its cells. From what I gather, you need to create a database. This requires some knowledge of Relational Database Management Systems. I also see that you need to create a User Interface. This requires knowledge of Application Development.

I suggest you breakdown your project into phases. In order for others to be able to help you, you need to understand how to ask a question that is relative to something specific. Right now, I do not see a specific question. I see that you have a need for a custom application.
