    Need help creating a form for one record with multiple items

    I am helping a friend create a log for a free furniture ministry. We would like to record one name and then list the items given to the client. The database includes Last Name, First Name, Referral Type, Item Given and Remarks. Often one client receives multiple furniture items such as Table, Couch, Chair. We didn't want to have to type the name multiple times for each item given. I am familiar with databases and queries but not sure about a subform in a form (if that is the best route to go).

    Create a related 'child' table for the client items.
