Different parts/rows of the same animal displayed in one from

i have in tables different mesurements of different teeth for example of one animal. i want to display this information one one form like in the image:


with the id number on top, and i want to display teeth 1, 2, 3, 4, ect. in the table these are in different rows. but have the same individual number ofcourse.

can anyone help me please,

Your data is normalized so cannot do that in a bound form used for data entry. In fact, denormalizing the data will not be simple.

this from is linked from a nested from, so this form is not nested in an other form. just a hyperlink between them

so what is another way to get what i want?

An unbound form but then that would require a lot of code to save data and to load existing data.
