Unhappy One Form, three entries, new records

I am working on a form that will request references from the user. It has field that are blocked together to make one new record. But how do I get the 2nd and 3rd blocks to create a new record as well? I am relatively new to some of the features in Access 2013. If anyone could assist that would be great. I appreciate the time and cooperation in this issue. This is the form below. Every time the users tabs into the Name field it should create a new record in the table associated with this form. Again, thank you for the assistance.

SubForms or a continuous form might work.

That form is not a normalized structure. If you want separate records for each name/address then remove the 2nd and 3rd sets. Enable the record navigation bar at bottom of form. Use the the arrows to navigate, even to move to a new record. Or allow the form to automatically cycle to new record after entering data in the last textbox. That behavior can occur regardless of form mode (single, continuous, datasheet).
