    Form not allowing new entry

Hello, it's been several years since I've delved deep into Access but at one point I was fairly good at it. Obviously that is no longer the case I have 2 tables joined and a query that pulls all the fields together, I then based a form off of that query, that I need others to input data into. I'm using a switchboard form for the sake of the users. The problem is that it isn't allowing for new records to be added. I know I screwed up somewhere, but I can't remember how I did this in the past. Any suggestions? Thanks so much!!!

I should add, that the properties in the data entry form is set to yes under allow data entry. thanks!
