I am writing to ask you help and support regarding transmittal form.Considering your expertise and experience in this matter, I believe you would be the best person to assist me. I am currently tasked with preparing a transmittal form using Microsoft Access. Please see below sample. I am very much thankful for your assistance and support.

Methinks you will need a report to act as the transmittal form. If you don't know how to create a report, try using the report wizard.

Have you even built a database for this purpose?
