Trying to figure out how to set a table in a form

I have a form that has date as the primary key. and bound to that, a table structured 10 rows by 5 columns. The first two values in each row remains the same, only the values in the last 3 columns to be filled in. I have tried to use a combo box, but I don't want a drop down, I would like an actual table structure with the required values visible in the first tow columns of each row for every date.

I currently have a table with date as primary key
table with all fields (joined date to date)
table with the values in the first tow columns

Can someone give me a pointer on how to resolve this?

Here is what I am thinking:

    Column1 column 2 column 3 column 4 column 5
    visible visible empty empty empty

you do this in a form,
the form can set the defaults you need in the text boxes.
Combo boxes are perfect for allowing ONLY the items required for that box.
