How do I get a query to appear within a report?

My boss now wants a query to appear within a report. I have the report set up:

Name:	report.png
Views:	15
Size:	4.8 KB
ID:	30476

This data changes, depending on the model entered in the form, and can be just a few lines as shown lines here or up to 18 lines.

    The form:

Name:	form.png
Views:	15
Size:	13.4 KB
ID:	30478

    The data:

Name:	data.png
Views:	15
Size:	10.2 KB
ID:	30477

    The query:

Name:	Query design.png
Views:	15
Size:	16.0 KB
ID:	30479

He wants the information to appear like this:

Name:	Final report.png
Views:	15
Size:	55.8 KB
ID:	30480

In the report design, I've tried to enter the query as a sub-form or as a list box. In either case, no information appears. I mean it is blank.

    Any step-by-step instructions?

You seem to have done appropriate steps to display the data. If you want to provide db for analysis, follow instructions at bottom of my post.
How to attach file: http://www.accessforums.net/showthread.php?t=70301 To provide db: copy, remove confidential data, run compact & repair, zip w/Windows Compression.
