Chart with Multiple Series from Query

I have a query - that filters the data using combo box from form by the person and year
There are multiple Tasks (Repair, Reports, Checkups) that I want to show as lines on Line Chart

There is CountOfCompletionDate for the numerical Y axis

There is Month for the X axis

The Query Shows the desired results in DataSheet view BUT the Form has blank area where chart should be.

Possibly you need a CROSSTAB query.

I analyze chart issues best when I can work with data. If you want to provide db, follow instructions at bottom of my post.
