When attachments are applied - my table links the document to two different incidents / areas

I have a table called Incident Escalation -

It is intended to help record & report system incidents to management.

Intermittently, when I add attachments to the incidents / cases - the attachment will link itself to more than one case.

For example - I added a .pdf attachment to incident #35 - and the .pdf was also found attached to #583.

Just to be sure - I removed & re-attached the .pdf - and the document still links to both cases.

Has anyone ever heard of these or similar issues?

    Any help is greatly appreciated

No idea what is happening. If you want to provide db for analysis, follow instructions at bottom of my post.

Embedding files, especially large images, will quickly use up Access 2GB size limit.
How to attach file: http://www.accessforums.net/showthread.php?t=70301 To provide db: copy, remove confidential data, run compact & repair, zip w/Windows Compression.

Please reply to this thread with any new information or opinions.
