Add new Record button doesnt work

The add new record button is not working for my application. I have a form that has a cascade combobox. Then i have some data entry fields. I need some code so that when a button is clicked it will add the data into a table and that it will clear the form for the next use.

Are you using unbound form? If so, why?

Data entry/edit to a bound form is committed to table when form closes, move to another record, run code. Instead of 'clearing' form, move to new record row.
