Printing Form that allows only certain edits.

I haven't made this form because I am trying to get everyone's opinion. . .
The company I work for prints there own labels. . .
    We have 3 different sizes 1x3 2x3 4x6 . . .
I want to make 3 separate forms that have multiple sections that can be chosen . . .
Upon Request I can Supply each separate Label with arrows to show what fields need to be changed for each Sales order.

For the 1x3 Labels we Make:
    1. ETL
    - We change model #
    - We Change if the Labels Says Dry Damp or Wet Listed.
    2. MISC.
-One Plain Txt Box that can be edited in the center

    For the 2x3 Labels:
    1.Box Labels
    -Customer
    -Item
    -Sales Order#
    -Shipping Location

(For the Box Labels I was thinking I have a Table that holds all this data and all I would have to make is a Query then a form)
    2.ETL
    -Change Model
    -Wet Damp or Dry
    3.CSA
    -Change Model
-Change the Wattage (3 Different Places)
    -Wet Damp or Dry
    4.MISC
-6 Text Boxes (From top to Bottom not side by side)

    4x6 Labels:
    1.HID Box Labels
    -Item
    -Watts
    -Color
    -Medium or Mogul Based

These are just all the details on the labels that we have to be able to change if you could make a database that would allow this I would be happy to supply you with a picture of each Label so that you would have the information.

    THANKS,
    You guys are great!

So in trying to understand your post's title - yes it makes sense that the form (for data input) reflects just the fields appropriate to the type label (data output).... Is there a further question?

Yes could there be a certain way to set up the form that would allow the user a preview and could I get a couple screen shots of a sample style form???

It sounds like you could accomplish this with a single form.
The form would have unbound text boxes for the various inputs.
It would also have a combobox with the three available label sizes.
Using the AfteUpdate event of the combobox, you would enable/disable the appropriate unbound text boxes.
You could also have a print button to execute a print preview of appropriate label.
