My form Disappeared!!!

I have been working for FIVE HOURS on my input form.
I am new to Access and am very excited to learn... but now I am so frustrated that my ears have turned red!!!

I was creating linkages... and creating a module to get a button to pull up a file path and when I went to view my form in "Form View"... all I see is the HEADER!!!

PLEASE HELP!

Does the rest of it show in design view? If so, you likely have a record source that doesn't return any records or allow new records.
