Need a report to use form/query output for data

I have a little DB here where I've got a form running a query against a DB that returns specific results based on the form input.

What I desire is to make a repot that is more pretty (that includes the fields I want on the form to post) to result in including ONLY the data presented from user input in the form.

e.g.- If you type "asthma" in the 'healine' section of the form, you return ~5 results (of the ~550 total in the DB). And now I need to understand how to run a report off of those 5 results only, so as to make the result more pretty like.

Can someone help me please?

Thanks June. It didn't work at first but I just started over using the form wizard first, then did what was on the website. Then it seemed to work fine.
