Question When selecting a value for one field how do you get other fields to auto populate

I have a form and a user selects the Activity Type. Each activity type has an activity # associated with it. I want the Item # to populate automatically when the Activity Type is chosen. I am pulling this info from another table that stores all the info, which I use as a lookup. Thank you
