I have data in a table. I created a form so my user's can enter date and after a chart pops up. I would like to view the chart in reports so i can e-mail it. Is this possible????

By chart do you mean a graph? Graphs can be built in reports. I don't know if it will email. Might have to save report as a PDF and send the PDF.

Yes Graph. I want my user to enter the from date and to date than the graph pops up in reports.
Is the Graph linked to data on the report? Can do this with Master/Child link properties or by WHERE clause in the graph's RecordSource. Example refers to field of the report's RecordSource to filter the graph data: SELECT Size, Pass, Hi, Lo FROM GraphBMD WHERE LabNum=[tbxLabNum] ORDER BY Size. I haven't use the Master/Child links but probably could and not need the WHERE clause.
So, filter the report's RecordSource and the graph data will synchronize.
However, none of my graphs use pivoted/aggregate data, but am taking for granted the synchronization will still work.

I have a form where the users enter the provider name and date of service. Base on the information enter it pops up a form with the pivot chart. I need it to be able to email it and print it.
So do you have the graph operating properly?
Although not intended to be printed objects, forms can be printed. It is my understanding forms can also be imbedded in a report in a subform/report container, although I never have tried.
If a report with embedded pivot form can be emailed is beyond my experience. I have tested emailing Access report as an attachment and it works. See http://www.techonthenet.com/access/reports/email.php
