Dynamic Check Boxes values from table

Hi,

I am using Windows 7 & Office 2010.

I have a table with 3 fields.

I wanna create a form which creates Dynamic Check Boxes as the values from a table.

If there is 3 values in my table then I need to have 3 Check Boxes and when I click them I need to do 3 different operations.

In my case I have a table_main_group and table_sub_group ( 1 to many)

Based on user clicks the check boxes I need to bring the child_form which contains the value from table_sub_group - again I need text boxes based on the values in the sub_group.



Please guide me.
