Help starting up a daily entry database

I need help creating a database that will allow a person to entry a daily report on equipment status, enter scrap for the production, shift, date etc. Along with this reporting I want al three shifts to have selection to select shift on form and they enter if the machines were down/up, report any stopage, scrap, issues with shift and submit report. each shift will enter a report daily. Can anyone help me get started on this. I would really appreciate it. Thanks.
