Form to add data to table only.

I have a table and a form.

Table--CIC/CUIGroupSRR
Form-- AcurenSRRForm

I want the form to be used to simply put data in the table.
When I close the form and reopen it I would like the form to be blank again. Not showing the data that was previously entered. Can this be done?

On the form properties' Data tab, set the "Data Entry" option to Yes.
