Access VBA Editor Customizations Lost After Exiting Access

I frequently add the code block comment/uncomment buttons to the Access VBA editor toolbar, but each time I exit and restart Access and the VBA editor the custom buttons disappear and I can find no option either in Access or VBA editor that would cause them to be retained. What am I missing?

    And you saved before exit?
and this was not in a subForm? (This has an odd effect sometimes)
and the db was not read only? (Other users in it)

He/she is referring to customizing the vbe command bar.
All I can think of is whether or not you're adding them properly. My bar options have been customized for ages and I've never had this issue. I also added a few debug options, outdenting (if that's a word) and indenting.
