To have all 20 (I think I counted correctly) Ticket types appear in the report, they MUST be entered into the "History" table. Looking at the PDFs for Arthur Carrot, it appears that "DL Number", "CBUS", "LSL", "ACIRT","CTAS",... are in the "Subset" field.
To have the Type/Subset appear on the report, it must exist as a row in the History table.
The fields "Card/Ticked Num", "Completed/Issued Date" and "Expiry Date" can be empty/NULL.

Does this make sense?

BTW, what does "Medical Date" represent?
