Name and DOB exact match query

Hello. I am a novice with access.
I have table which I want to create a query where I want to gather the results of a name and dob which both need to be an exact match from the linked form.
What would I need to put in the criteria section of the query form under the name and dob fields??

in the query critieria, use the BUILDER. (ellipsis wand on the toolbar)
it lets you pick the exact item on the form for criteria. (currentdb, openform, form...)

    like: forms!fMyForm!txtDob

You want to have a form in which you will enter name and dob and then you want to see any records that match

    Is this correct?

See the attached file for one example of how you might do this. Post back if you have any questions.
