No idea. But i don't use the design wizards.
What happens if you manually build with Form or Report Design instead?

Not that I have ever used the wizard but it does sound like something happened to your installation if it previously worked. I would try running a repair of your Office installation (or if you just have Access run a repair on it.) Also make sure you have all your updates.
It may also be that your application is corrupted.
First make a backup then decompile to remove any corrupt compilation code, then recompile and compact

I've tried everything suggest here & nothing is working to fix this problem. i have just created this database & it has very little information in it. When i pick the button to create a form or a report Access closes. i'm not very experienced with Access, but i know something is wrong with the program itself. Does anybody have any other ideas? Thanks for all your help.
Hmm, an old issue but may be pertinent. Have you checked to make sure the location of your wizards are trusted? You can check that here...
File -> Options -> Trust Center -> Trust Center Settings -> Trusted locations
I suggest you upload a copy of the db. The issue might not be related to the suggestions and we might notice something.

You know another thought. What is your bitness 32 bit or 64 bit? It makes a difference as we may be all testing in the wrong bitness. (Had that happen once already this month!)
