    create a lookup on multiple fields prior to creating new record

    I'm new to access and need help on this. I created a crew database and want to find out if a new entry may already be there. (They change names, but may use the same tax id, email address, contact name, phone number or postal address)


    I want to create a form where I can enter 1 up to 5 of the previous elements and click a button to check if any of them are already present.
    Additionally, I'd like to save time by using what I already typed to create a new record if no duplicates are present.

    Otherwise, enter data into bound controls and use code behind each input control that does lookup for that single or multiple values. Code in the example can be adapted.
