I'm not clear on the question. If you want a notes field, you'd add one to the underlying table and then bind a textbox to it.
I agree with Paul --unsure of the question/intent.
Do you have a statement of requirements or similar that you could post?

If you want each note to be its own record then you need another related and dependent table and a subform for data entry.

Just set the Form View to Single Form
