Is Access the right application for this?

I'm looking for an application that will allow me to create a database which allows my users to make selections and based on those selections, it could spit out a Word document. The selections are tied to various pieces of text and possibly tables that would populate the Word document. It sounds simple, but the document I'm trying to generate can get complicated. Can Access accomplish this and if so, do I need Access 2010 or could I do this with Access 2007? If not, is there another application which could do this?

Access can be set up to allow users to make selections and based on those selections either create a report or mailmerge with a Word Template. Each combination of selections would need to have a corresponding result from either a yes/no field to a drop down selection box and use of free text would need to be regulated somehow. If the document can be templated than access could fill the alternative info.

Attached a sample doc and someone will answer with more details or provide sample DB.

I meant to say for you to load a copy of the document and someone would maybe produce a sample DB that does the job.
