Combo box with multiple choices and check boxes - Help.

I am using a form and used combo box, can somebody explain on how item 4 and 5 was made possible.

I am using groups in design view but only 1 item at a time will appear as a result. In this sample if you check 2 box or more , the names will appear in the action box.

This is made possible by having the table allow multiple values in a field. Go to table design view, go to the Action field, open the "Lookup" tab below, set "Allow Multiple Values" to Yes.
