Input mask issues

Hi there,
I'm creating several input masks for some fields in a form.
The problem is that when I access the Customize Input Mask Wizard, and fill out the information (see Input Mask.png); then click Close, I get the following message: "Field cannot be empty."
It wasn't empty when I clicked on the button. Now, I can't change the Sample Data and I can't get out of the Wizard.

Hi June7,
I tried to attach it again and the Computer tab is not working. The image is showing right now in my original [I edited it to include the image after my itchy trigger finger did its job]
