i need a help i need to Capture Modified By Whom & Modified Date If someone Modified the Existing Details right Now i my database is capturing person who enter the details..........

you'd have to provide a field in all your tables (assuming you're tracking only the most recent update) and with any update/add function append the username (which you say you're already capturing) and the date (and/or time) of the event.

You can capture the date with the date() function
you can capture the date/time with the now() function
