Fields do not show in "Form View"

I have created a query based upon a table that has no data, yet. The table is designed as I would like. I then created a form (using the query) to use to input the data required. Everything look OK in "Design View" however, when I switch to "Form View" there are no data entry fields showing. I am obviously doing something wrong.

I started out using this same method on other db's and had no problem with tables, queries, forms, or reports.

This one has me stumped. Thank in advance for any and all help.

Do you have AllowAdditions set to YES?

Talking Fields do not show in "Form View"

I found the problem. When I created the query I included two tables and set up the wrong join.
