easy checkbox question

How do i link a series of checkboxes to a table so that the first check box text populates with the first row first and last name in table employee. the second check box text populates with the second row of first and last name in table employee?

I am not entirely sure what you mean, but I am guessing that the form view type you will want is continuous.
