    Enter the value in the form and show results (sort of like query but not query)

    What I want to do is that I want to create the form and every single time a person enters the last name or patient ID, it should show contact information of the patient, when they had appointment and so on. It shouldn't be a query.

    Hello! Thanks for the video. It helped. What if there are two customers with same last name? How will that pop up?

    In your SQL statement for the lookup, add the first name. In the properties for the combo box, set the width of the combo box to include the first name field. Both names will appear and you can then select the one you want.
