Drop-down list values selection

All,

I am requesting if anyone could help me with a drop-down list issue located in a sub form within a form. Instructions are located in the compress folder and instructions are in text and images which makes it easier to understand. Basically what I am trying to solve is when a user selects a certain value in a drop-down list in a field, the user will be able to select only a few values from the other drop-down list in another field.
