Running a query from another DB as a subform

I have a form and built a query from another data base. I have inserted the query as a sub form on the other data base. I need to run the query based on an Incident number that is entered into the form, this would then just display the incidents that have been logged that day and or associated.

Right now I have used the "Control Wizard" and at one point it prompted me to link form fields from the primary form, I did that and when entering the incident number it does run the query but a window pops up asking for the number again, after it enter that it runs the query and displays the data but adds another record with the same number and no corralating data. I just want the query to be READ only and if there is not an incident associated then nothing should appear in the query.

    Make sense?

Here is the option that prompted me to link the form fields
