Building a database from scratch

Hi all



I need to build an Access database for work. I have used it before and know the basics but cannot get my head around linking forms.

Now, what I have at the moment is 1 table with names of GP Consorias and their addresses and contact details.

What I need to do, is link GP surgeries with the relevant Consortia, each Consortia has around 50 GP surgeries and I need to be able to search for all GP's in a specific consortia.

I also need to be able to search by region - so I can see all the GP's surgeries in a specific region (East mids, London etc...)

Can anyone help? Below is a screen grab of what I have done so far.
