    Just to clarify, I didn't mean the diagramming thing so much. You'll find that e/r is explained in the normalization pages. If they don't do it for you, there are tons of other ones you can check out.

    Table Relationships links make no sense.
    Client text field is linked to AppDate date/time field.
    ClientID autonumber is linked to Client text.

    When building queries, they will try to establish links built in Relationships window. Linked fields are not compatible so causes type mismatch error when opening query, or report.
    No wonder you are frustrated. Better fix relationships before proceding.

    In addition to the sites that Micron suggested, you might also go to "Rogers Access Library"and work through the 3 tutorials. The trick is - you really have to follow along in Access - actually creating the dBs.

    I see at least 3 more tables....
    And you need to create forms for data entry. Should (almost) never enter data through tables.

    Thanks for the feedback. Between Google and this forum, I have tried everything that I KNOW and sadly it just isn't working. I really wanted to simplify the way information is reported in the office and was ecstatic about it. I am now overwhelmed and frustrated.

    I gave it a good try...and I appreciate the help.

    I did, but this is my first time using Access 2016. I'm not sure how to re-establish the relationships. I really wanted to knock my boss's socks off with this new development in tomorrow morning's staff meeting. LOL!

    Fix them how? I am lost on what should connect where. The ClientID is the unique number that links everything else. I thought that was the "magic number" to link everything so nothing gets lost.

    There are no shortcuts here. Read the material. Follow tutorials and build alongside them. It's apparent that you don't understand the basics, which means telling you how to fix it MIGHT help this time but what about next time? You must understand entities and relationships first. It's not the foundation, it's the footing.

    Have a look at your database with my modifications. Look at Relationships window first. Then open the report.

    So the records AND the choices should've been linked in the Relationships tab? Oh!!!!!!!!! Still reviewing but just had to note that.

    I think i am about to cry!

    Thanks! Now I can update the report design to include all aspects since some of the text boxes are too short for all that it contains.

    My boss is going to be SO FREAKING AMAZED! They are still keeping these records in Excel...................
