That took a minute. There's an embedded image on the form.
Hmmm .... how do I see/extract the image that was embedded on the form? I am just trying to understand this form.

Not sure. I deleted it and the color went away, so that told me where it was.
The only forms that I have seen to exhibit this header color scheme is in the Microsoft supplied template databases. I have in the past, searched Google for information, and not come up with anything specific, just some suggestions that crudely duplicate the effect. It's a Microsoft mystery.

I found the following. I made a copy of the Sales Reports Dialog form because the steps below would not work with an unbound form. I have attached the image. Thank you pbaldy for steering me in the right direction. I am fairly new to Access and did not know that you could use an image in this way.
This is an embedded background image? According to Access Junkie at http://www.accessjunkie.com/Pages/faq2010_77.aspx the process is:
1. Take your form that has a background image or AutoFormat that you like
2. Right-click on the form in the Navigation Pane and select Export >> XML
3. On the first page of the wizard, select a destination to drop some files (Desktop is fine)
4. A small dialog will then appear with three check boxes
5. Select the first option (Data) and third option (Presentation) and then click OK
6. Click the Close button on the last page of the wizard after the export is complete
7. Navigate to where you dropped the files and, among other things, you'll find a new folder called Images
8. Open that folder to claim your prize
