Club membership db design-how to list members-by person or membership

When designing a club membership db with two types of memberships, how should the members be listed? My club has Individual and Family memberships, the latter with two persons, some with the same last name and some with different last names. One method:

TBLMemberships
MemshpID, FirstName1,LastName1,FirstName2,LastName2,Address, City,ST,ZIP,Phone1,Phone2,Email1,Email2 Where this method is used the Individual members would have no input in the fields designated with a 2.

At some point, I will have to have another table/s that designate:

• membership type
• newsletter preference - digital or paper
• record of each year that each person has been a member
• dues payment dates
• amounts paid
• receipt sent date

that may serve as a starting point.
It is generic in that it deals with most common tables and attributes. You can add to/delete from or adjust or ignore.
