Request Form Template with Attachments

Hello All!!

I was wondering if there's any request form template I can use and pattern a project management tool I am trying to create?

I would like to be able to
send a request --> send email confirmation (with request #) ---> allow users to send updates/changes to request #

rather a vague description of what is required, but access has templates and if you google something like 'access project management templates' you should find plenty more as per this link
