How to create a field on a form that becomes a hyperlink if condition is true.

I have a form with a field that asks if the record as an associated label. If Yes I would like the text to become a hyperlink and when clicked on would open the file.
The URL to the PDF ,which is stored on SharePoint, is located in another table. The relationship between the two tables would be between fields SDS# and SDS Country.
I would like to know the best way to achieve this.

Have two fields, set Visible to yes or no depending on the value of Label.
