searching a select group of records with a form

I am build a database that has mutiple forms writing data to the same table, the forms are set up for data entry. Each form has some different fields on it. My end user also wants a copy of each form that they can use for editing records after they are entered if needed. is there a way to set up a search box that will only look at records that were created with a specific form?

youd have to save the form name into the record.
