    How do I rename a form?

    I don't know what I've done or how I've done it but the name of my form which appears on the tab along the top is now looking something like this:



    = Extended <<Expr>> [Main Table]![Extended]

    Extended is the name of one of my check boxes in the form. The form should just be named Case Details but I was playing around with the Extended field this morning and now this has appeared.

    Does anyone know what I've done or how I can change it back?
