Populating a form based on 3 parameters

I have a table called Main. 3 of the fields are called firstname, lastname and academy. I want to be able to key these in and for Access to retrieve and display all the other fields relating to this unique record in my form. i've tried recordsource using a query but that displays as a datasheet. any suggestions would be helpful. the table contains about 5000 records/rows. thanks

    5000 records isn't an issue.

Sooo many ways to skin this one.
Make a query that takes parameters.
    or
Make fields in the Header/Footer of the form and then Filtering the Contents based on that input.

hi, thanks for that. i've got 3 unbound fields in the header, and I've got a query that selects * from my table. in my query design I can assign each of the input fields to the corresponding column in the table. not sure how to proceed.
